Cares Community Health, dba One Community Health (OCH), was founded in 1989 as a 501c (3) in response to the HIV/AIDS epidemic affecting some of the most indigent and socially isolated individuals in the community. Since 2012, OCH has re-focused and expanded its mission, striving to offer a full continuum of primary care services to the community at large, becoming a Federally Qualified Health Center (FQHC) in 2015. OCH is headquartered in Sacramento, California with the following vision and mission, “We envision a health community that compassionately cares for all,” and “As one community, we empower all people through equitable, quality, and patient-centered care.” OCH’s service area spans approximately 965 square miles and includes 28 zip code tabulation areas (ZCTAs), 27 of which are in Sacramento County and the remaining ZCTA is in Placer County to the northeast. Cities represented in the service area include Sacramento, Carmichael, North Highlands, Citrus Heights, Antelope, Fair Oaks, Orangevale, and Granite Bay. The service area is entirely urban, with an estimated total population of 991,837. OCH’s target population includes individuals and families with incomes at or below 200% of the Federal Poverty Guidelines (FPG), the uninsured and underinsured, and those who face barriers to care. The service area’s target population is comprised of 337,348 residents, representing 34% of the total population. The target population is defined as low-income, uninsured, and underinsured individuals including those covered by Medicaid and Medicare, and people without housing.

Despite the challenges of the COVID-19 pandemic, OCH experienced uninterrupted and notable growth in its patient population – growing 41% in five-years, from 13,102 unduplicated patients in 2019 to 18,516 unduplicated patients in 2023, below are demographic statistics about the 2023 OCH patient population:

  • Ethnicity: 26% were Hispanic, 64% were non-Hispanic, and 10% were Unreported/ Chose Not to Disclose
  • Race: 14% were Asian, 16% were Black/ African American, 42% were White, 24% were Unreported/ Chose Not to Disclose, 2% were More Than 1 Race, 1% were Native Hawaiian/ Other Pacific Islander, and 1% American Indian/ Native Alaskan
  • Gender: 47.3% were Male, 46.8%. were Female, 2.6% were Transgender or Nonbinary, and 3.3% were Chose Not to Disclose or Unknown

Income Level: 74% lived at or below 200% of the FPG, 8% lived above 200% of the FPG, and 18% were Unknown.
