El Dorado Community Health Center

El Dorado County Community Health Center ("EDCCHC" or the "Center") is a 501(c)(3) non-profit corporation. The Center has been designated by the U.S. Health and Human Services Administration as the Federally Qualified Community Health Center (FQHC) for El Dorado County. The Center is not affiliated with any governmental unit or entity. The Center operates three full-time medical clinics located at 4212 Missouri Flat Road in Placerville, and 3104 Ponte Morino Drive, Ste. 110, Cameron Park, California as well as one full-time medical and dental clinic located at 3100 Ponte Morino Drive, Ste. 120, Cameron Park, California. EDCCHC provides primary medical care, behavioral healthcare, eye care, and dental care for the medically under-served and under-insured populations of El Dorado County. The Center operates all clinics as primary medical care facilities utilizing an integrated behavioral health model for the provision of psychological services. Primary medical care provided includes all normal family medical practice examinations and procedures. These services include care for acute and chronic illnesses, wound care, immunizations, well baby and well woman examinations, routine check ups of all sorts and family planning. Most specialty medical services are provided by referral to specialist physicians.

The Center also offers a broad range of Patient Support services based on the needs of our patients and the requests of our medical providers. These services include: discounted prescriptions, case management, outreach, eligibility assistance, medical referral assistance, and health education. The Center currently employs a staff of 160, including physicians, nurse practitioners/physician assistants, podiatrist, psychologists, licensed clinical social workers, and psychiatrist. The Center's physicians are certified by the American Board of Family Practice, and properly licensed by the State of California.  All of the Center's healthcare professionals have been selected to reflect the highest standards of the medical industry with a focus on providing a medical home to the most vulnerable population of El Dorado County and surrounding areas.

Our mission is to improve the health of our community through quality healing and preventive services.

Needs

Over the last 20 years, El Dorado Community Health Center has made great strides in establishing access to preventative healthcare to our vulnerable population in the county. The center currently serves nearly 12,000 patients which includes those with private insurance, Medicare, and Medi-Cal as well as those who are without insurance. Access to care is impacted by site capacity and has increased health center locations which causes disconnection in services for patients. Patients need a true health home by creating a center with integrated services.

Our newest center opened in 2022 located on Missouri Flat Road, in Placerville, CA. The center is fully functioning preventive care services with accessibility to public transportation. Our vision in 2023 is to bring expanded health care services in one central location for our patient population who many times lack the ability to travel long distances in the County. These services include primary care, behavioral health, pharmacy, dental, and community services in one location to serve all of El Dorado County.

Without El Dorado Community health center many chronic illnesses would continue to go undiagnosed until emergency treatment is needed. Patients build trust with established providers and familiar faces. EDCHC provides that stability for patients.
